Use: Use this form to change your name on your records at the University. Use it regardless of your Relationship to the University--Student, Employee (Administrator, Faculty, or Staff), Alumnus or Alumna, Friend, or Donor.

Use: Use this form to authorize the release of your education record to another person (ex., a parent) or agency other than the University of Memphis and its employees.
Note: DO NOT USE THIS FORM TO REQUEST AN OFFICIAL TRANSCRIPT. (Use the Request for Offical Transcript form instead.)

Use: Use the form to apply for GRADUATE credit by examination.

Use: Use the form to apply for UNDERGRADUATE credit by examination.

Form: FERPA Form for non-Student System Users

Use: Use this form if you are either: (1) a student employee who needs to access student data to perform your University duties or (2) an enrolled student who would like to request access to student data for legitimate research purposes.

Use: Use this form to change one of these attendance dates for a student if the online grading period has ended. If the grading period is still open, change the date online instead.

Use: Use this form to change a student's grade after the online grading period has ended. If the grading period is still open, change the grade online instead.

Use: Use this form to assign a grade to a student who attended your class but whose name did not appear on your class list by the grading period.

Use: Use this form to extend an "I" grade for a student.

Form: Request for Approval to Register After Late Registration

Use: Use this form to approve a student's request to register for classes after the last day of Late Registration has passed.
